NDIS Provider Program Licensing & Use Agreement

1. Program Use and Forward Sale Only

I acknowledge that:

• The programs provided are designed to be sold forward to participants as a support service.
• I am not permitted to sell, share, or transfer the materials to another provider, organisation, or individual outside my service.
• I will use the program for delivery to NDIS participants only, in line with the support structure described at the time of purchase.

2. Copyright and Intellectual Property

I agree that:

• All program content, including slides, workbooks, scripts, and supporting resources, remains the intellectual property of Life Skills Better Connections.
• I will not copy, modify, reproduce, or distribute any part of the program content without prior written consent.
• I will not rebrand or sell the content under a different name or business.

3. Permitted Use

I understand I am permitted to:

• Use the purchased program for unlimited delivery to my participants under my organisation.
• Charge participants for program delivery as part of my NDIS-funded support
• Deliver programs in both group and 1:1 setting.

4. Confidentiality

I agree to:

• Keep all materials secure within my organisation
• Ensure that only authorised team members involved in program delivery have access
• Not upload or make content publicly available (e.g. on websites, social media, or shared drives)

5. Acknowledgement

By signing this agreement, I confirm that I have read, understood, and agreed to the above terms. I understand that failure to comply may result in the cancellation of my program license and potential legal action for breach of copyright.
